Recent changes on how GP services are provided were announced by NHS England earlier this year. Under the new 10 year NHS Transformation plans, all GP practices are now required to deliver services to a patient population of between 30k - 50k patients. These new services will be delivered by newly formed PRIMARY CARE NETWORKS (PCN'S). PCN'S will work to deliver new and additional services, focusing on the specific needs of each unique patient population in collaboration with other local health service providers (such as Lancashire Teaching Hospitals, Lancashire Care and local charities.

Our network has a total patient population of over 38k patients and includes all the practices within our group:

Patients will soon be able to access newly developed services and access appointments across our wider network and as these become available we will communicate them via our websites, in surgery waiting rooms and if appropriate via direct mail.
Evening and Weekend Access
Giving you More Choice
As part of our newly formed Primary Care Network - evening and weekend appointments will now be available to all patients at more locations.
From September, we will me making it easier for you to make appointments:
Monday - Friday: 6.30pm - 8.00pm
Saturdays: 8am - 12 noon
Sundays: 8am - 11am
Bank Holidays: 8am - 12 noon
(Christmas Day - Closed)
If you do need to see someone, we will be able to offer you an appointment at the local 'hub' location for that day. This will ordinarily be at either:

- The Surgery Chorley
- Withnell Surgery
- Whittle Surgery
- Clayton Brook Surgery
- Dr Hamad Surgery
- Dr Jacobs Surgery
There, you will be seen by one of the Network's local-based healthcare professionals - the service is led by GPs but also includes other staff such as nurses and healthcare assistants. The staff will have full access to your medical records, ensuring a high quality service. Full details are available by viewing our privacy notice.
Please note - the extended opening hours is not a walk-in service - you need to make an appointment.
You can make an appointment by talking to our surgery staff and booking a slot in the normal way (including online via patient access), or- if we are closed and you think you need to see someone more urgently - you can call NHS 111.
As with all appointments, there are limited numbers available. If there are no appointments free, and if you feel you are unable to wait until a routine appointment at another time, then you should contact 111 or visit the out of hours service.
If you feel you have a medical emergency, you should call 999.
